Insurance & labs

"Do you take insurance?"

We're a cash-pay practice by design — it's what lets us spend 45–90 minutes with you instead of twelve. Here's what that means for labs: routine labs your insurance already covers (CBC, metabolic panel, standard thyroid, A1c) can be run through your regular doctor or insurance-billed at a national lab, and we're glad to work with recent results. The advanced panels that drive your protocol — comprehensive hormones, hormone-metabolite testing, gut analysis, genetics — aren't covered by any insurance plan, anywhere. They're built into your consult and program pricing at below-retail rates.

Using outside lab results — and the $150 lab credit

We accept outside labs when they are within 90 days of your consult and complete per our requisition list. If results are older or missing key markers, a redraw is required — protocols built on stale or partial data aren't protocols, they're guesses. Your care coordinator will provide the requisition list before your consult so you know exactly what's needed.

Have recent comprehensive labs from another provider? If your panel is within 90 days and complete per our requisition, a $150 lab credit applies to your consult — mention it when you book. If your panel is close but missing a few markers, we'll draw only the gaps at cost.

How we choose your testing

We source testing from multiple accredited specialty laboratories and match the specific panel to your presentation — not the other way around. No single vendor dictates your workup. Rebekah selects the panel that answers the clinical question your case is asking.

Prescription continuity

Safe prescribing requires current data. Refills are not sent if visits or labs are not current — no exceptions. Minimums by patient type:

Program patients don't track minimums — visits and labs are built into the program and scheduled for you. Minimums apply to patients maintaining prescriptions outside a program. Fee-for-service is appropriate for stable, established patients on unchanged protocols.

Portal messaging

Portal messaging is for refill requests and logistics. Clinical questions, side effects, dosing adjustments, and medical concerns require a scheduled visit — because your health deserves more than a message-length answer. Thrive members receive responses within the response window of their plan; program patients are seen monthly, so most questions are handled in visit.

Programs & billing

What your consult includes — and how prescriptions begin

Your consult delivers your complete written protocol — your labs interpreted line by line, your treatment roadmap, and Rebekah's clinical recommendations. That protocol is yours to keep, whatever you decide next. Prescriptions from your protocol are initiated inside your treatment program, where ongoing titration, monitoring labs, and monthly visits make prescribing safe and effective.

Maintenance options — the Thrive Membership and fee-for-service care — are available once you've completed a treatment program. This keeps every patient on a path that gets them stable before shifting to lower-touch care.

One exception, for established transfer patients: if you're transferring from another hormone clinic, stable on an unchanged protocol for at least 6 months, you may begin with a Foundation Consult and move directly to a Thrive Membership or fee-for-service care — no program required. Records from your prior clinic are required at intake to confirm your treatment history. Bring your recent labs as well: if your panel is within 90 days and complete per our requisition, the $150 lab credit applies to your consult. Ask your coordinator whether you qualify.

Treatment program terms

The Kickstart (12-week hormone program) and Repair & Rebalance (24-week gut + hormone program) are billed monthly for the full program term. Programs are a clinical commitment with a defined endpoint — enrollment is for the complete term, and included labs are scheduled by Rebekah within the program timeline. Program pricing is reviewed with you at booking and at your consult. Consult enrollment credits ($100 Foundation, $200 Optimization, $300 Precision) apply toward your first program month when you enroll within 7 days of your consult.

Maintenance paths — three options, matched to you

All maintenance paths are available once you've completed a treatment program — or, for established transfer patients (stable on an unchanged protocol for 6+ months, with prior-clinic records at intake), directly after a Foundation Consult. Which path fits depends on your therapy and the level of access you want:

Thrive Membership is the best fit for patients on injectable testosterone — the medication cost is efficient enough to include, making the membership an all-in value.

Elevate Membership is concierge-level care for patients who want more — monthly visits, direct access to Rebekah, and a higher-touch experience. Requires additional screening; not available to every applicant.

Fee-for-service is the best fit for patients on BHRT (estrogen patches, progesterone, vaginal estrogen) or anyone who prefers to pay per visit. Your coordinator will point you to the right path based on your treatment.

Thrive Membership terms

Month-to-month, with 30-day written cancellation notice. Includes quarterly visits, monitoring labs (as clinically indicated), prescription refills handled through the portal, priority scheduling, member pricing on IV therapy and supplements, and one premium wellness IV each quarter delivered at your quarterly visit (does not roll over). For patients on injectable testosterone, the cost of your prescribed medication is included when clinically appropriate and continuity requirements are met — prescriptions always follow Rebekah's clinical judgment and current labs, never the membership alone. Membership pricing is provided at your consult and at program completion.

Elevate Membership terms — concierge care

Elevate is Restorative's highest-touch membership, designed for patients who want direct access, monthly oversight, and a fully managed care experience.

  • Monthly visits (in-person or telehealth)
  • Advanced lab testing included
  • Monthly premium wellness IV included
  • Direct text access to Rebekah
  • Appointment availability outside standard clinic hours
  • Injectable testosterone prescription included when clinically appropriate

Membership pricing is provided at your consultation. Elevate requires additional screening and is not available to every applicant. Prescriptions always follow Rebekah's clinical judgment and current labs, never the membership alone.

Fee-for-service pricing

For patients maintaining care outside a membership:

  • Follow-up visit (30 min) — $225
  • Telehealth visit (20 min) — $125 · care plan review, treatment adjustments between scheduled visits; out-of-cycle reviews conducted via telehealth only
  • Acute telehealth visit (15 min) — $69 · sinus infections, UTI symptoms, non-emergent acute care only; no protocol reviews or lab panels
  • Basic hormone panel (cash-pay) — $125
  • Comprehensive lab panel (cash-pay) — $299

Fee-for-service care carries the continuity minimums above and is available in open schedule gaps; program and membership patients receive priority scheduling at all times. Lab reviews during scheduled visits are included at no charge; out-of-cycle reviews are conducted via telehealth only and billed at the 30-minute telehealth rate.

Mid-program changes

Sometimes the picture changes mid-program — a new symptom, a new finding, a new goal. When it does, Rebekah will discuss options with you directly at your next visit, including whether additional testing or a program adjustment makes sense. These are clinical conversations, not menu items.

If something comes up that feels too pressing to wait until your next scheduled visit, you're welcome to book a 20-minute telehealth visit ($125) to address it sooner.

IV therapy

All IV therapy requires a good faith medical exam prior to treatment, per Oklahoma Board of Nursing guidelines effective March 2026. Exams are valid for one year and are telehealth-friendly for established patients. Good faith exam: $35 initial · $20 health status update. IV memberships carry a 3-month minimum enrollment, then month-to-month with 30-day cancellation notice; unused monthly IVs do not roll over.
